Ordering by Fax...
Faxed orders using a credit card for payment must include the following:
l. List of products desired by name, quantity, stock number, and price(s).
2. Your name as it appears on your card, credit card billing address, complete card number and expiration date.
3. Your complete delivery address(7.75% sales tax will be added for Calif. residents) and any special instructions(such as "leave package at suite #123 if addressee not present")
4. Shipping method desired. UPS Ground shipping is the standard shipping method for ActionMD customers.
5. Fax, telephone number, and e-mail address if available.
Faxed orders will normally be processed by the next business day. ActionMD will total your order and fax or e-mail confirmation
If faxing an order and paying by personal check, certified funds, or money order, your order will be held until we receive payment. Personal checks must clear before orders are shipped.
